I started on Survival difficulty. The differences which I know are: Enemies do 2x damage. Player character does 50% damage to enemies. There are more Legendary spawns. Health regens A LOT slower when eating/drinking/using Stimpacks. You get better loot because more legendary enemies spawn. May require multiple playthroughs if you f*ck up and miss out on the miss-able trophies. Overall there are tons of guides, fantastic game with a very rewarding platinum. They're very different, so it depends on what you like. Very hard is the normal game, but your guns don't do damage, while the enemy damage gets massively boosted. Other than that it's just like playing normally. Survival on the other hand tweaks things massively, so the game feels very different to play.
