Aug 11, 2017 · Combined IM/anesthesia is a dumb idea and a waste of time. It may improve your knowledge, but it does not improve your chances of getting a job. It is impractical. You will end up practicing one or the other. If you try to be both a PCP and an anesthesiologist, you will just end up being mediocre at both. You should consider the work life balance. The work life balance even as an attending surgeon is not going to be as good as an attending anesthesiologist. The hours are still longer and is demanding. I wouldn't put too much thought into the years required for residency. Even in anesthesia, you'll need intern year plus CA1-3.

As far as Anesthesiology vs. EM, I would say EM has better "lifestyle" b/c they work less hours (probably an average of 35 vs. an average of 50 or so) and they are never on call (barring mass casualty or you have to cover for a sick co-worker). I didn't do my fellowship at Cleveland Clinic, but I am a big believer in workhorse, non-supervisory, do-the-goddamn-cases-yourself, cardiac anesthesiology fellowships (as opposed to echo fellowships). It boggles my mind when I get on here and see people asking where they can go train and work as little as possible.
